That depends on who's using the terms.
For most of the DNS world, a primary server gets its domain data from
its own files, while a secondary server gets its domain data from
some other server. A server that is primary for one domain can be
secondary for another. Using a secondary server instead of multiple
primary servers for a domain is simply a way to reduce the
administrative work required to update the domain.
When interacting with domain registrars, the primary server is the
one you enter first, while the secondary is the one you enter second.
They actually don't care which is which.
Once domain data is loaded into both primary and secondary, the
difference disappears. They are functionally equivalent.

Yes, a secondary is a separate machine. Under the terms of the
license agreement for QuickDNS Pro, you can run two copies, providing
that one copy is used only as a secondary (backup) for the other one.

This is something others can answer. The official recommendation is,
whenever necessary.
I just restarted my local DNS server (also providing other services)
for the first time in several months. The problem wasn't QuickDNS,
but the TCP/IP stack, which had become corrupted. Nothing crashed,
but the machine was deaf to the network.
