Go to Edit Salespeople in maintenance, and make sure you have salespeople
setup.

No... You may want to do away with Pending accounts altogether if you
don't need them... (If you are receiving payments on a pending account,
you already have accepted them as a customer).

Completed is the default button label... But Done was used for places
that Completed wouldn't fit.
