I have a similar problem (or at least closely related) with load balancing
not working.
Many many weeks ago, and merely as a test, I had set up a load-balancing
record for one of the domains we host (I had never used this before).
Well, it worked (more or less).
Then we had some important changes in our server set-up, and I forgot all
about this load balancing (also because our main server was pretty stable)
(QuickDNS 3.01 runs on another machine). Eventually, I shut down the second
(for testing only) server, but still did not think of removing (as I said, I
forgot all about it) the load balancing record (which was set to 99%-1%,
BTW)
But at a given moment, the server (W* 4.3 /G4 /OS 9.04) started crashing
regularly (say, once every two days) (not related to DNS). While looking for
answers, we decided to install Kick-Off (great product, if I may say so).
BUT ! While the server was rebooting, the load-balancing did it's job (at
least, it tried), but as you guess, with the second server being down,
without success. This results in the domain being inaccessible (as if the
DNS server crashed).
Schematically: server goes down, load-balancing switches to non-existent
backup-server, then hangs (but not totally) ..., even after the main server
is back up !
How I found out ? All other domains remain accessible via the DNS, and the
particular site is accessible through it's IP address, but no longer via the
domain name. Only a reset of the DNS server (restart) solves the problem.

Rgds,
Nick
