Nope... setup fee are one-time fees. Not recurring, and therefore do not
show up on recurring invoices.

You could add it to the billing cycle window, but it would invoice them
for it everytime an invoice was generated for them until you took it out.

Well, generally speaking, ISPs invoice customers at the time they setup
their account (not a month down the road).
