'Invoice Type For Credit Card/Debit Accounts' is set to what?
'Invoice Type For Other Accounts' is set to what?

When you click the Generate Invoices button, that's really all it's based
on... Unless you have overridden that setting on a per customer basis
(check the customer's Invoices screen).
