You're using the Mac OS X (Carbonized) version of QuickDNS Manager?
Look in your preferences folder (~/Library/Preferences/, where "~" is
your home folder) for a file named QuickDNS Manager Server List.
Either trash it or, if you're feeling really brave, edit it with
BBEdit.
