In the real world this is what happens:

Customer signs up
Account creation fails (for whatever reason)
support/billing person on phone sounds like an idiot becuase OG says
everything is OK (or worse customer on phone thinks we are idiots for not
getting it right the first time).
ANOTHER person has to get involved to fix the problem or at least look into
it (as the operator will have not idea what the SQL failed e-mail means
anyway).

So now there are two people involved, a customer on the phone and OG
telling everyone that it worked.

A Better Solution(tm) would be:

Customer signs up
Account creation fails BUT A BOX POPS UP SAYING IT FAILED.
Before the customer has a chance to call in we fix the problem and the
customer is happy and we look like billing and Internet GODS.

Unless you audit your OG and radius/e-mail/ftp servers monthly I can
garuntee that you have 1. dead accounts 2 accounts that are active but not
being billed or 3 accounts that are not active but being billed (ie
additional e-mail accounts, ftp space, domains).

OG simply has to be able to return error codes to the operator when things
fail.
