I went to Maintenance & Preferences -> General Preferences -> Accounts
and selected "No" on "Use Pending Accounts". I assume this will not
send any new customer to pending account. What happens if the
customer's credit card is declined. We would like for it to go to
pending in this situation.

You need to have the system not send them to pending... Pending means
they are pending someone's approval. You can turn off pending accounts
under Maintenance & Preferences -> General Preferences
