QuickDNS 4.5.1 for Mac OS X has a problem (in the installer, not the
software itself). This will be fixed soon. Anyone who installed
QuickDNS Remote as a fresh install (not an upgrade) within the last
week or so and is now experiencing the same problem (can't add the
name server) should contact Men & Mice technical support.

As for the second question, this has already been answered privately.
In short, the answer is, once the new server(s) is/are set up, tell
the ISP to take down their old version of the zone.
