Okay, I went ahead and ripped it all apart, and everything seems to be
working just fine.
Check the following preferences:
Create Separate Usage Invoices (set it to 'No')
All Usage Goes To Single Invoice (set it to 'Yes')
Both preferences are on the Usage Preferences screen.
