Okay, I think I tracked down what's going on... CCAuthorize seems to be
returning the wrong code for PSiGate (and it looks like a few other gateways
that CCAuthorize supports).

Optigold triggers the sequential decline routines based on the code
CCAuthorize returns. The 3 valid codes it can return are:

1 = Approval
2 = Decline
3 = Error

Optigold triggers the sub-routine if it's a code 2, but *not* a code 3.
Since 3 is *supposed* to mean there is some error with the gateway.

I have a ticket into the manufacturer of CCAuthorize, so hopefully they can
get an updated CCAuthorize.jar file out (it won't require an update for
Optigold).
