I know about the bank name requirement for AuthorizeNet. Since
AuthorizeNet ACH stuff was a last minute addition to 1.8.9, the bank name
portion wasn't able to be added in time for the release. But it's
already part of the 1.9 release.

Actually that's not true... a debit card can be used as a credit card if
it has a Visa (or other) symbol on it. In actuality, they are not really
"traditional" debit cards. They are simply a Visa card that extracts
money from the bank account in realtime. So they are just "tricky" Visa
cards.
It was done that way to maintain compatibility with existing systems. As
far as the authorization systems are concerned, it's just a Visa.
Yes, a Bank Debit is more along the lines of an ACH.
